    Registration Information

    Lincoln High School encourages all students to explore and participate in our athletic program.  Lincoln embraces the student-athlete approach to our program and works closely with our coaches and students in support of their success both on and off the field. 

    Athletic registration for the 2019-2020 school year will be offered online!  Athletes and families will complete all paperwork, upload your physical, and pay ASB through our online portal, FamilyID (www.familyid.com).  Online registration will open in mid-February.  There will be an Athletics Online Registration Help night on Wednesday, May 1, 2019 from 6:30-8:30pm for any families who would like support or help with the online process. 

    Due dates for registration paperwork and the first day of practice for each sport are listed below. For the upcoming school year (2019-2020), interested athletes for any season are encouraged to register this spring. This will allow our coaches a better idea of interested athletes and provide initial contact information for informational meetings prior to each season. Any student-athlete interested in participating in any of our summer training opportunities must complete and turn in registration information and paperwork by the initial paperwork deadline Friday, June 21, 2019.

    All athletes need to turn in paperwork and must be cleared prior to participation. Athletes turning in late paperwork are not guaranteed a roster spot (due to equipment availability and for safety reasons in some sports the number of athletes may be limited).

    Students registering for any of our sports must complete and submit (all forms printable on SPS athletic web page):

    • Athletic Registration Packet
    • Physical Examination form (good for two years)
    • Sport Specific Guardian Release & Warning Forms
    • Guardian Awareness Forms
      • Extra Curricular Athletic Transportation Form
      • Weight and Fitness Room Form
      • Concussion and Sudden Cardiac Arrest Awareness Form
    • Out of District Athlete (only if applicable) Form
    • Student Insurance Forms

    As part of registration, please make sure you read and review the Student Athletic Handbook. All athletes and participating families are responsible for the guidelines outlined in the Athletic Handbook.
